Step 2: Schedule a Tour 

  • Experience our facility firsthand!
  • Meet our team, explore our programs, and see if we’re the right fit for you.
  • Schedule a visit using the form here or by giving us a call.

Step 3: Speak with Your Service Coordinator

  • If you receive services through a regional center, inform your Service Coordinator about your interest in Central Valley Adult Care.
  • They will help determine eligibility and guide you through the referral process.

Step 4: Complete the Enrollment Packet

  • Download and fill out the Enrollment Packet.
  • Submit the completed forms via email, fax, or in person.
