Step 3: Speak with Your Service Coordinator
- If you receive services through a regional center, inform your Service Coordinator about your interest in Central Valley Adult Care.
- They will help determine eligibility and guide you through the referral process.
Step 4: Complete the Enrollment Packet
- Download and fill out the Enrollment Packet.
- Submit the completed forms via email, fax, or in person.
